Mario the owner was there, he is fantastic. Catered to us . Has been there for 30 Years there is a reason the food was wonderful.
A small restaurant, but one of the best in San Francisco. I have been coming there for years. The host and hostess, Mario and Stephanie, are fantastic, warm, and engaging people. The food is outstanding and authentic Southern Italian cuisine. I have have been fortunate to have eaten food throughout Italy, and eating at Caffe Macaroni brings tears to my eyes. I have a habit of ordering their Tricolore Salad and Positano (black linguine with prawns, mussels, clams, calamari, baby octopus and garlic). Their Sanguine is superb and be careful of Stephanie's Dolce! If you make a decision to share it with your friend/date, you will be cast as a selfish, greedy, narcissists! You will eat more than your proper portion! I have recommended and have been accompanied by people from all over California and the entire globe. Most recently, from folks who live in Australia!

Excellent food, the octopus was the most tender and flavorful I have EVER had, the antelope was wonderful, and the cheesecake was to die for. The best meal I've had in months.

I've eaten here a number of times over the years, and the food is always great. From a simple linguini agile e olio, to the seafood risotto, everything is fresh and tasty. Most recently, I had a dish of very fresh halibut, accompanied by a serving of escarole, with antipasto of calamari and cannellini beans. With the cozy, homey ambiance, and the hospitality of Mario & Stephanie (the owners), one feels like a dinner guest in their home, not merely a customer at the restaurant. The wine list is ample yet not overwhelming, and has entries for individual glasses, moderately-priced bottles, and premium bottles, for that occasional splurge. The location is a few blocks from the more tourist-oriented zone, but well worth the detour.
I have been coming here for over 25 years and the limoncello cake is the bomb! My friends and I shared the following: the creamy mozzarella and arugula salad, followed by the eggplant parmigiana, linguine di mare and then the porchetta. We also shared a slice of the limoncello cake and the panna cotte with persimmons. I would come just for the limoncello cake but the food and the people have always been great. It is a not so hidden gem at the edge of FiDi/Chinatown/Little Italy. However, the owner, Mario, is thinking of retiring so better enjoy this while it lasts.
They have never disappointed us! We've been going to Cafe Macaroni off and on for about 6 years now. We haven't been able to go for about 2 years now and it's like seeing an old friend. Great food and great service!
Caffe Macaroni reminds me of the small mom and pop Italian North Beach restaurants I remember from the 1970's. The cafe is quirky, very red, and decorated in a whimsical manner. The restaurant is very tiny. We sat upstairs and we both ducked our heads. This area is intimate and cozy. You become quick friends with whoever you are seated by. The fennel and orange salad was outstanding. My husband had the tricolor salad which was also super delicious. I had the halibut on eggplant and my husband had the pork dish. The eggplant was sublime. The halibut didn't taste as fresh as I had hoped and my husband felt the same way about the pork. I side of "pizzaola" pasta was as good as my husband's Italian grandma made from scratch.
Limoncello cake split for dessert. Vermentino wine quite good. We will return.
We had a reservation for 7:15 on Valentine’s Day. The restaurant was way too crowded, and we were not seated until 7:50. We did not receive drinks until 8:15, nor food until 8:30. The food ended up being good, but the wait kind of killed the mood.
Caffe Macaroni is always outstanding. The food in this tiny place is the best italian you will find in San Francisco and the atmosphere is always intimate and unique. Great place for a date, to take out of town friends or business associates, it never fails to impress.

Adorable place with excellent food. Particularly enjoyed the black pasta with mussels, clams, shrimp, and calamari. My friend had tuna and that was good too. Suggest reservations because the place is tiny.

Great, cozy place! We had the meatballs and the seafood linguine. Both were amazing. It would be hard to have more than 4 or so people, and a reservation is highly recommended. Especially if you must sit inside. But very cute and cozy. Great for a rainy December day!

Delicious food, wonderful wait staff and an authentic North Beach experience. The black squid ink pasta seafood special was excellent. Very small restaurant that encourages you to talk to nearby tables; if that's ok with you, it'll be a good time.

Owners Mario and stefania are the real thing. They provide authentic Napolitano cuisine. Even the menu is written in Napoletano dialect.

Wonderful memories of everything about Caffe Macaroni from years of being able to just drop in after work or at lunch. Now it's a destination choice for us but just as charming and delicious as previously. Mario and the fellows that back him up are just great.
Recently there was an article circulating about the sad state of affairs for restaurants in SF -- just based upon how busy Caffe Macaroni was from 5:30p to 7p -- clearly they've risen above the fray!

If you want a quieter experience, ask to sit upstairs. If you want to be in the heart of things, ask for upstairs or the sidewalk.
We love Caffe Macaroni. It is best to make reservations as it is such a small little place. I would suggest that you not waste your money on the glasses of wine (go for a bottle or no wine at all) as it is really poor quality. Perhaps the worst wine I've ever had. But the food is absolutely delicious!
